Draft of insurance waiver.... my first crack at it. Please review ALL the words to ensure that I do not have something messed up. We can delete the unnecessary and add what is needed.

looks good, but I would move your tables over a bit on the "print name" part... that should be smaller because the sig one also has the date in there. Also if someone signs this on Saturday, is it good for sunday as well? or would they need to sign a new form with a new date? Do they really need to sign the date if the whole weekend is listed in the top right of the waiver?

Good for the weekend... from when they sign until the end of the event.

I think date is needed to show when they sign it.

I can make the name, signature, date a single row instead of trying to do two columns.
